About the Role
- Conduct assessments and develop personalized service plans based on client needs.
- Collaborate with clients, families, and interdisciplinary teams to coordinate care and ensure effective service delivery.
- Facilitate access to community resources, therapy, medical care, housing, and employment assistance.
- Monitor client progress, adjust service plans as needed, and ensure all goals are met.
- Advocate for clients and provide ongoing support to help them navigate complex systems.
- Maintain accurate and timely documentation, including progress notes, assessments, and service plans.
- Attend and facilitate team meetings, case consultations, and coordination with external providers.
- Foster positive relationships with clients, service providers, and community resources.
Requirements
Bachelor’s degree in Social Work, Psychology, Human Services, or related field (Master’s degree preferred).
Prior experience in service facilitation, case management, or related positions.
Strong understanding of mental health and substance use disorders, including trauma-informed care.
Excellent communication, problem-solving, and organizational skills.
Ability to work independently and collaboratively with a team.
Compassionate, empathetic, and client-centered approach.
Preferred Qualifications:
Knowledge of local community resources and social service agencies.
Familiarity with Medicaid, mental health services, and client rights.
Experience working with diverse populations and culturally competent care.
