About the Role

- Conduct assessments and develop personalized service plans based on client needs.
- Collaborate with clients, families, and interdisciplinary teams to coordinate care and ensure effective service delivery.
- Facilitate access to community resources, therapy, medical care, housing, and employment assistance.
- Monitor client progress, adjust service plans as needed, and ensure all goals are met.
- Advocate for clients and provide ongoing support to help them navigate complex systems.
- Maintain accurate and timely documentation, including progress notes, assessments, and service plans.
- Attend and facilitate team meetings, case consultations, and coordination with external providers.
- Foster positive relationships with clients, service providers, and community resources.

Requirements

  • Bachelor’s degree in Social Work, Psychology, Human Services, or related field (Master’s degree preferred).


  • Prior experience in service facilitation, case management, or related positions.


  • Strong understanding of mental health and substance use disorders, including trauma-informed care.


  • Excellent communication, problem-solving, and organizational skills.


  • Ability to work independently and collaboratively with a team.


  • Compassionate, empathetic, and client-centered approach.

Preferred Qualifications:


  • Knowledge of local community resources and social service agencies.


  • Familiarity with Medicaid, mental health services, and client rights.


  • Experience working with diverse populations and culturally competent care.
